Independent provider directory monitoring

See which provider directory records require review, and why.

We read what health plans publish and check every listed NPI against the federal deactivation and exclusion registries.

Method v1.0
Complete

14,803 of 14,803 provider-directory pairs carry a federal event date, each one provider in one directory. The Index counts the listings behind them, which runs higher because a provider can appear in several files of the same directory. Measured against NPPES_Deactivated_NPI_Report_081026_V2.zip and the HHS-OIG LEIE published 2026-08-10, retrieved 2026-08-26.

The Rollcall Index

A market-wide measure of directory integrity.

One rate for the market: qualified federal findings for every 10,000 directory records read. Zero is the target, and every point above it is a record a plan can go and fix.

Every contributing directory stays visible, so a plan can put its own rate beside the market without a difference in coverage disappearing into an average.

Rate
Qualified findings per 10,000 records evaluated
Band
What the rate implies once file size is accounted for
No score
A directory we could not read, never a zero

The rate covers only the checks named in the current method version. It is not a measure of overall directory accuracy, and it is not a compliance determination.

Each node is one published directory in the run you are looking at, ordered by its own rate. A node gives the band its directory falls in and does not name it: the ring is drawn in rate order, so a label on each node would make this an ordered league table of every constituent. 1 directory could not be read and carries no score; a directory that could not be read is not the same as one scoring zero. 9 directories are showing the measurement they carried forward, last measured in 2026-W34 to 2026-W35, which is outside the Index above.

Read 30 August 2026 and published 30 August 2026. 25,422 qualified listing records in 16,315,452 evaluated. This is the current run.

Why this runs continuously

Published directories change all year; a periodic review is stale before it is finished. We read every published directory weekly, against dated federal records.

Why auditing needs to be continuous · The method behind every number

Continuous directory integrity monitoring

An external integrity control for published provider directories.

Rollcall re-reads the directory you publish and compares it with authoritative federal sources, week after week. It raises the discrepancies that qualify, keeps the evidence behind each one, and hands them back in a form your team can work from.

Your last line of defence

An outside-in check that continues after the directory is published.

Independent
No internal roster required, and nothing asked of your systems
Continuous
A pass every week, which is as fast as the federal record moves
Reviewable
A published method, every check recorded, and a finding you can dispute

The federal files behind the checks are monthly, so a weekly pass is as fast as the evidence moves.

How a finding is produced: the directory a plan publishes and the federal status records are read into the Rollcall audit engine, which retrieves and normalises both, applies the published evidence gates, preserves the audit trail, and is operated by an agent that reviews every run before anything is released. Qualified findings carry a state of new, persistent, resolved or recurrent, and leave on a steady beat, delivered over the API, by webhook, or as a downloadable file.
