Each county page lists the medical QHP issuers certified to sell there and the provider directory file each one registers with CMS.
A state publishes counties only when every one of its Individual-market medical issuer registrations resolves to a directory in the currently published Index. Alaska, North Carolina, Oregon, Texas are not published here: at least one issuer selling in those states registers a directory the Index has not yet read, and a county list with a silent hole reads as complete. What this page is not
- It is not a plan recommendation. No premium, no ranking, and no result of any kind: issuers are listed alphabetically. Costs and coverage live on HealthCare.gov, which is where a plan should be chosen.
- It carries no Rollcall Health result. The audit measures published directory files and reports one market-wide level; nothing published about the current run says what was found in any named plan’s file, and a plan receives its own findings on request.
- It is not a statement about any doctor. Nothing here says whether a listing in a directory is right, and nothing here is about any clinician.
- It is not a network claim. A directory listing a doctor is not the doctor accepting the plan, and a doctor absent from it is not out of network. Rollcall Health does not resolve either question, and does not try.
- The host is not the plan. Directories are often published by a vendor or a shared platform. The registration maps an issuer to a file; it does not say who operates the server.
