One page per insurer brand: the states it sells Individual-market medical plans in, the issuer registrations behind them, and the directory file each registration points at.
Registrations from CMS's public plan-year 2026 files. A brand page carries no rate and no band: a finding is a property of a published file, and it is reported to whoever publishes that file. A registration maps an issuer to the directory file it publishes under 45 CFR 156.230(b)(2). The host serving that file may be a vendor or a shared platform; the registration does not say who operates the server, and none of this is a statement about directory accuracy or network participation. If a directory listing misled you
A wrong listing is reportable, and the regulators below exist to hear it. Include what happened, the plan name, the provider, and the dates.
- Your state insurance regulator. Every state insurance regulator takes consumer complaints; find yours through the NAIC state directory.
- The No Surprises Help Desk. If you relied on a directory listing and were billed as out of network, federal law may protect you: call 1-800-985-3059.
- The Marketplace. For problems with a HealthCare.gov plan or enrollment, call 1-800-318-2596.
