Where the audit lives

Rollcall audits the directory file a host publishes, not the legal entity that registers it. This page carries no rate and no band, and nothing published about the current run does: the Index reports the market without identifying the files in it. The records and read date beside each host say how much was read and when, never what was found in it. How a brand is tied to a file, and what that chain does not claim, is written down in the attribution methodology.

A registration maps an issuer to the directory file it publishes under 45 CFR 156.230(b)(2). The host serving that file may be a vendor or a shared platform; the registration does not say who operates the server, and none of this is a statement about directory accuracy or network participation.

The public record

• The HHS Office of Inspector General found that 22% of maternal-health providers listed in Centene's Medicaid managed-care directories told OIG they were not in-network, and 35% of its in-network providers were missing from the directory entirely.
